I have read and agree to the Johnson Motor Works Terms of Service (johnsonmotorworksnc.com/terms.html).
I identified the part(s) to be replaced. Johnson Motor Works does not diagnose problems and makes no promise the repair will fix any issue.
All labor and any parts are provided "as is" with no warranty. Customer-supplied parts are my responsibility.
Once the work is completed and I accept my vehicle, Johnson Motor Works is not responsible for any later repairs, failures or damage, and its total liability is limited to the labor amount I paid.
I have inspected the work and accept my vehicle. I release Johnson Motor Works from any further claims about this service, except any claim I report in writing within 7 days; any lawsuit must be filed within 7 days of completion, or the shortest period allowed by law.
I had permission to have work performed at the service location.
